Exogenous BHB Ketones: What They Do, and What They Don't
The short version
Beta-hydroxybutyrate is a real fuel your liver makes from fat when carbohydrate is scarce, and swallowing it in salt form genuinely raises the amount circulating in your blood for two to three hours. What that does not tell you is where the energy came from. Nutritional ketosis is a state where your own fat is being converted; exogenous ketosis is a delivery. Blood tests cannot tell the two apart, which is exactly why the marketing does not either.

What a ketone actually is
When carbohydrate runs short, your liver converts fatty acids into ketone bodies — mainly beta-hydroxybutyrate and acetoacetate — and releases them as an alternative fuel that muscle and brain can both use. This is ordinary physiology, not an exotic state. It happens overnight, during a long fast, and continuously on a ketogenic diet.
The key point is that in this normal version, a high blood ketone level is a symptom. It is evidence that fat is being broken down, because fat breakdown is the only way those molecules got there.
What an exogenous ketone does to that logic
An exogenous ketone supplement contains BHB itself, usually bound to a mineral to make a stable, palatable salt. Drink it and BHB is absorbed and appears in your blood within about an hour, peaking and then clearing over roughly two to three hours. That much is well documented in controlled human studies.
But now the symptom has been produced directly. Your blood ketones are elevated and your fat stores have not moved. The measurement that used to be evidence of fat oxidation has been decoupled from it — and no meter you can buy can tell the difference.
On a ketogenic diet, high blood ketones mean you are burning fat. After a ketone drink, high blood ketones mean you drank ketones. Same number, different event.
The feedback loop nobody advertises
There is a second-order effect that makes the marketing claim worse rather than merely incomplete. Ketone production in the liver is regulated, and circulating ketones feed back to suppress it — partly directly, partly by nudging insulin up and free fatty acid release down.
So during the hours after an exogenous ketone drink, your own ketone production tends to fall. For someone whose goal is a ketone reading, that is invisible. For someone whose goal is burning their own fat, it is pointing the wrong way. This is not fringe: it is a consistent finding in the human metabolism studies, and it is the single most important thing to understand about the category.
What exogenous ketones are genuinely good for
Appetite. This is the strongest claim and it holds up reasonably. Raised blood BHB is associated with lower ghrelin and reduced subjective hunger for a few hours. If hunger is what breaks your diet at four in the afternoon, that is a real and useful tool.
The transition. The first week of low-carb eating is unpleasant largely for electrolyte reasons, and BHB salts arrive with sodium, magnesium and calcium attached. Some people also find the ketone itself smooths the energy dip while their metabolism adapts.
Fuel and cognition. There is genuine research interest in ketones as an exercise fuel and in cognition, particularly in older adults. It is early, mixed, and not what a weight-loss product is selling.

Will a ketone meter show a difference?
Yes — that is the point people find persuasive. A blood or breath meter will read higher after a BHB drink. The reading is real and its cause is the drink, not your fat stores.
Are exogenous ketones useless then?
No. Appetite suppression for a few hours is genuinely useful if hunger is what derails you, and there is interesting work on exercise fuel and on cognition. They are just not a fat-loss mechanism, which is what they are usually sold as.
Do the salts matter?
